At every appointment, we kindly ask that you present your Alberta Health Care card and Photo ID upon checking in with reception. This process is done to ensure that we have the correct person booked in our computer system, and to confirm that the Alberta Health Care number is valid. This process also helps to prevent identity theft and fraud.

Upon checking in, we will also ask you to confirm your current address and phone number. This will assist us in keeping open communication with you to provide high quality care.

 

LATE OR MISSED APPOINTMENTS:

Sylvan Family Health Centre understands that your time is valuable, as is ours. In consideration of our other patients we ask that you show up for your appointment a few minutes early. When we reserve time for you, the physician requires all of that time to provide you with the best quality of care possible. When you are late it decreases our ability to provide this for you. If you arrive 10 minutes late or more to your appointment you will likely be asked to reschedule unless the physician’s schedule can still accommodate you. The receptionist will speak with the physician to see if you can still be seen. Priority will be given to the patients who arrive on time and you may have to be worked in between them. This may mean you could have a considerable wait, and you may not have the full 15 minutes that your appointment allotted you. One or two late patients can cause the entire daily schedule to fall behind.  This is an inconvenience to everyone. We strive to see every patient as close to their appointment time as possible. 

If you think you will be late, or are unable to make your appointment, please contact the clinic as soon as possible. If you do not show up for your appointment, we will mark you as a No Show. Three No Shows with your physician in a calendar year will result in a $55 charge. If your appointment is with one of our PCN staff, there is a $5 fee charged for each appointment you miss.

We ask that you please be courteous of your providers’ valuable time and attention.  The physicians, office staff, as well as your fellow patient’s thank you.

 

NEW PATIENTS:

At this time, we are not accepting new patients to our family practice.

We are currently accepting prenatal patients. If you are pregnant and are needing a physician to follow you throughout your pregnancy, please give the clinic a call. You do not need a referral from your family doctor, nor will you need a "Meet and Greet" for a prenatal appointment.

We offer the following specialized services, accessible to our patients, as well as those who are not part of our family practice. An initial consultation is required to determine if you are a good candidate for these services. Please call our office for more information.

  • Botox Injections
  • Circumcision
  • Prenatal Care & Delivery
  • Vasectomy

If you have been accepted as a new patient to our family practice at Sylvan Family Health Centre, we will schedule an initial “Meet & Greet” appointment with your family doctor. This appointment is an opportunity for your family doctor to become familiar with your medical history, and to answer any questions you may have about our clinic and access to health care in Sylvan Lake.

If you are unable to attend your appointment, please give us a call and we will be happy to reschedule. Please note that there is a $55 fee if you miss your “Meet & Greet” appointment. This must be paid before your appointment can be rescheduled.
