“Forge a path. Change a life.” This is the vision that drives our commitment to promote the health and well-being of our community by providing accessible, compassionate, high quality care, in a collaborative environment.
Sylvan Family Health Centre is an innovative family medicine practice located in the picturesque community of Sylvan Lake, Alberta. Our team is composed of a variety of health care professionals, who each contribute a unique skill set to serve our family of patients. Every day, we strive to build an individual connection and trust with each of our patients.
When visiting our clinic, you can expect to see a variety of learners journeying with us on the path to providing our community with high quality care. Mentorship is highly valued at Sylvan Family Health Centre, as we work with family medicine residents and medical students to guide them on their career path. Their ultimate goal is to serve you, and we gladly work alongside them to nurture and support their developing skills.
We are passionate about being leaders in the delivery of primary medical care, and demonstrating a model of care that addresses the multifaceted needs of our patients. As a team, we have the capability to address needs in relation to:
- Physical health
- Mental Health
- Disease and Screening Prevention
- Lifestyle changes
As we forge ahead, on a path directed towards excellence, we hope to change the lives of those we serve.
For those who have not heard, Dr. Julie Hernberger is moving her practice to Saint Mary Family & Walk-In Clinic in Red Deer. The physicians and staff at Sylvan Family Health wish her all the best in her new adventure. If you are a patient of Dr. Hernberger and you did not receive notification of this move please call our clinic to be informed of what your options are. As always, we will continue to provide care to our patients through this transition.
The physicians and staff of Sylvan Family Health Centre wish to inform you that we will no longer be providing a Saturday Clinic to our patients. With the opening of the Advanced Ambulatory Care Centre, we realize that our need for a Saturday clinic will continue to diminish as we have already noticed a decline in the number of patients we see on a Saturday. All URGENT but non-life threatening medical needs can be seen at the Ambulatory Care Centre. For life threatening situations, please call 911 immediately. All other appointments can be booked with your physician throughout the week. Our last Saturday Clinic will be Saturday, June 23. We do apologize for any inconvenience this may cause.
Dr. Maciejko, and Dr. Penner are currently accepting new prenatal patients only. If you or someone you know is pregnant and are needing a physician to follow you through your pregnancy, please give the clinic a call at (403) 887-2224.
