At every appointment, we ask that you present your Alberta Health Care card and Photo ID. We will also ask you to confirm your current address and phone number. 

Late or Missed Appointments

In consideration of our other patients we ask that you show up for your appointment a few minutes early. If you arrive 10 minutes late or more to your appointment you will likely be asked to reschedule. We strive to see every patient as close to their appointment time as possible. 

If you think you will be late or are unable to make your appointment, please contact the clinic as soon as possible. If you do not show up for your appointment, we will mark you as a No Show. Three No Shows in a calendar year will result in a $55 charge. If your appointment is with one of our PCN staff, there is a $5 fee charged for each appointment you miss.

New Patients

At this time, we are no longer accepting new patients to our family practice.

All new patients at Sylvan Family Health Centre are scheduled an initial Clinic Orientation appointment with one of our nurses. This appointment is an opportunity for our nurse to become familiar with your medical history and to answer any questions you may have about our clinic. Once you have concluded this appointment you will be able to schedule an appointment with your new family physician. 

We offer the following specialized services, accessible to our patients, as well as those who are not part of our family practice. An initial consultation is required to determine if you are a good candidate for these services. Please call our office for more information.

  • Prenatal Care & Delivery
  • Nexplanon Birth Control Implant

Please note that there is a $55 fee if you miss your “Clinic Orientation” appointment. This must be paid before your appointment can be rescheduled.
