What is a Primary Care Network (PCN)?
In a Primary Care Network (PCN), a group of family doctors works with Alberta Health Services and other health professionals to coordinate the delivery of team based comprehensive services for their patients. Sylvan Family Health Centre is a member of the Wolf Creek PCN. This collaboration allows us to offer comprehensive services to our patient’s that are specific to the needs of Sylvan Lake. The health care professionals that we currently have working alongside us include:
- Diabetic Foot Care Nurse
- Family Nurse/Chronic Disease Nurse
- Mental Health Therapists
We offer several FREE sessions through the PCN for our patients. Currently, we are offering:
- Weight Wise: This program is focused on helping people achieve healthier weights for healthier lives. This course will provide participants with tools and information as well as access to nurses and dieticians to assist you in meeting your weight goals.
- Tobacco Cessation: If you currently use tobacco of any form and are interested in quitting, this program is for you. This program focuses on helping individuals move towards a tobacco free life. There are afternoon or evening sessions available.
- Heart Health: If you have high blood pressure or high cholesterol, this course is for you. Our dietician offers courses on improving and maintaining better results for blood pressure and cholesterol through diet and lifestyle changes.
